About The Position

TSG Health & Wellness, a telehealth program under The Ship Group Services LLC, provides integrated medical and behavioral health support for Medicaid and underserved communities. The role delivers telehealth primary care and medication management in an integrated care environment, with coordination between primary care and behavioral health clinicians. The position is PRN and begins with one to two consistent clinic days per week, with the possibility of expanding as patient volume grows. It is remote and requires authorization to practice in North Carolina. The Physician Assistant conducts visits for common acute and chronic primary care needs, manages medications and follow-up care when appropriate, uses behavioral health screening tools, orders and reviews labs and diagnostics within scope and payer requirements, and coordinates referrals and warm handoffs. Documentation must be completed the same day when preferred and within 24 hours at the latest. The clinician must follow telehealth standards, privacy requirements, and internal clinical protocols. The organization provides administrative support, including intake coordination, scheduling, reminders, non-clinical follow-ups, chart preparation, referral logistics, and clinical templates and workflows. Supervising physician support and a clear escalation pathway are in place; the clinician is not responsible for patient acquisition. Compensation is $45–$50 per hour for scheduled, patient-facing clinic time, based on experience. Compensation may be reviewed after six months based on program growth, volume, and quality metrics. There is an optional growth path into a Lead APP role as the program scales.
