About The Position

Aligned Modern Health (AMH) is a leading evidence-based holistic medicine practice with a strong presence in the Midwest and a rapidly expanding national telehealth service. We offer personalized care across various disciplines including functional medicine, hormone health, clinical nutrition, chiropractic care, rehabilitation, and acupuncture. Our approach focuses on identifying and treating the root causes of chronic conditions through advanced diagnostics and individualized treatments, aiming to empower lasting well-being. We are seeking licensed Chiropractic Doctors (DCs) to join our dynamic and growing functional medicine team, both locally in Chicago and virtually nationwide. AMH manages all aspects of practice management, including staffing, billing, and reimbursement, and provides necessary equipment. We also have a robust marketing team, and participation in content creation and events is expected.

Requirements

• At least two years of experience practicing functional medicine and performing functional medicine specialty testing/labs
• Experience in functional and integrative medicine settings preferred
• Must have (or be able to obtain) an IL license and any remote location determined
• Strong oral and written communication skills with the ability to collaborate and interact effectively with patients and other providers
• Energetic and outgoing personality with a desire to help people
• Patient first attitude and ability to consistently provide a 5-star level of service
• Knowledge of HIPAA regulations and ability to maintain patient confidentiality
• Active Illinois Chiropractic license required to start employment

• Extended work/training in areas such as American Academy of Anti-Aging Medicine (A4M), American College of Lifestyle Medicine (ACLM), Institute of Functional Medicine (IFM), Diplomate of the American Board of Chiropractic Internists (DABCI), Diplomate of the American Clinical Board of Nutrition (DACBN), Functional Medicine University (FMU), and the College of Integrative Medicine (CIM)

Responsibilities

• Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
• Develop personalized treatment plans based on patients' needs and medical conditions
• Prescribe nutraceuticals and/or medications and monitor their effectiveness
• Collaborate with other healthcare professionals to coordinate patient care
• Provide counseling and education to patients on disease prevention and healthy lifestyle choices
• Maintain accurate and up-to-date patient records using electronic health record systems
• Stay current with medical advancements, research, and clinical trials in the fields of functional, integrative and lifestyle medicine
