Requirements
- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in KY, MI, OH, or IL
- Consistently exhibits professionalism, integrity, accountability, confidentiality, care, and compassion.
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking.
- Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA secure messaging apps.
- Strong written and verbal communication skills.
- Ability to work independently or in a team environment.
- Pass national background check and valid clinical license search.
Responsibilities
- Review caseload and prepare technology for servicing patients.
- Navigate schedule using modern technology and partner with in-home clinical team members (Sprinters).
- Make connections with diverse patients, collect information, and perform various services.
- Conduct health risk assessments, medication reviews, cognitive screenings, and provide patient education.
- Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and perform necessary tasks.
- Provide comprehensive care coordination and management, including preventive care, medication management, referrals, and documentation.
