About The Position
Sprinter Health is seeking part-time, dynamic Nurse Practitioners with a New York license to revolutionize healthcare delivery by conducting virtual wellness visits directly to patients in the comfort of their homes. As a Nurse Practitioner with Sprinter Health, you'll leverage your medical expertise to offer a wide range of healthcare services that could include but not be limited to virtual adult and/or pediatric wellness visits, health assessments, and more. Successful candidates will have prior experience performing wellness visits, along with key traits such as dependability, professionalism, and problem-solving abilities. A commitment to delivering exceptional customer service is essential, as is the ability to work autonomously while maintaining high-quality standards. We're seeking individuals who are friendly, compassionate, empathetic, and deeply invested in providing personalized care to every patient they serve. The role involves reviewing caseloads, preparing technology, navigating schedules using modern technology, partnering with in-home clinical team members (Sprinters), making connections with diverse patients, conducting thorough health risk assessments, medication reviews, cognitive screenings, empowering patients with health education, evaluating vital signs with Sprinters, arranging blood draws, carrying out necessary tasks, providing comprehensive care coordination and management, including preventive care interventions, medication management, referrals to specialists, community resources, and documentation of findings.
Requirements
- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner License in New York
- Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search

Responsibilities
- Conduct virtual wellness visits to patients in their homes.
- Perform virtual adult and/or pediatric wellness visits.
- Conduct health assessments.
- Review health risk assessments, medication reviews, and cognitive screenings.
- Empower patients with educational information regarding their health and well-being.
- Work with Sprinters to evaluate vital signs, arrange blood draws, and carry out necessary tasks.
- Provide comprehensive care coordination and management.
- Implement preventive care interventions.
- Manage medications.
- Make referrals to specialists and community resources.
- Document findings.
