About The Position

Concierge Home Care is seeking an experienced Licensed Practical Nurse (LPN) with strong home health and triage experience to join their growing Virtual Care team. This role involves delivering exceptional patient care from home, focusing on innovation, compassion, and technology. The ideal candidate will thrive in a fast-paced clinical environment, enjoy helping high-risk patients remain safely at home, and desire a consistent weekend schedule while working remotely. The position is for Florida-based candidates only and involves providing proactive remote support and clinical intervention to reduce hospitalizations and improve patient outcomes.

Requirements

• Active, unencumbered Florida LPN license
• Minimum of 2 years of home health experience
• Experience with triage and hospitalization prevention
• Strong clinical judgment and communication skills
• Comfortable using telehealth platforms and EMR systems
• Reliable internet connection and private remote workspace

Nice To Haves

• Experience working with high-risk or medically complex patients
• Knowledge of chronic disease management
• Microsoft Office proficiency

Responsibilities

• Conduct virtual patient visits and clinical assessments
• Perform triage and identify early warning signs or risk trends
• Support chronic disease management and patient education
• Monitor telehealth data and escalate concerns appropriately
• Help prevent avoidable ER visits and rehospitalizations
• Collaborate with interdisciplinary care teams
• Document patient interactions accurately and timely
• Participate in care planning and performance improvement initiatives
• Support development of virtual care best practices
