About The Position

Provides executive leadership and accountability for the design, integration, and performance of patient care progression across the OSF Ministry. This role collaborates closely with the System Chief Medical Officer and Chief Financial Officer to advance clinically excellent, value-based care strategies; oversees capital, financial, and human resources within the care progression and care management portfolio; and fosters a culture of continuous improvement, innovation, learning, and accountability to reduce fragmentation and ensure patients receive the right care, in the right setting, at the right time.

Requirements

• Bachelor’s degree in nursing or social work
• Five to seven years of related experience required, including at least five years in a leadership role with responsibility for multi-site or enterprise clinical operations, care management, care coordination, population health, clinical transformation, or a related function.
• Active Illinois RN license, Michigan RN license, or Social Worker License, as validated by the issuing licensing board.
• Accredited Case Manager governed by the American Case Management Association or Certified Case Manager governed by the Commission for Case Manager Certification within two years from date of hire
• Excellent interpersonal and communication skills.
• Solid computer skills, including proficiency with Microsoft software.
• Strong analytical and problem-solving skills, with the ability to be detail oriented.
• Systems Thinking: Demonstrated ability to analyze, design, and optimize complex healthcare systems, recognizing interdependencies across clinical, operational, financial, digital, and community-based domains.
• Applies a systems-thinking approach to address root causes, reduce fragmentation, and improve care progression, patient outcomes, access, capacity, and stewardship of resources.
• Transformational and Innovation Leadership: Demonstrated ability to successfully lead large-scale transformation across diverse clinical, operational, and interdisciplinary teams. Ability to translate insights into operational action and measurable improvements.
• Data & Digital Fluency: Strong understanding of healthcare data, performance measurement, analytics, risk stratification, population health tools, and digital-health technologies, including predictive modeling, care-coordination platforms, electronic medical records, automation, and interoperability.
• Strategic Agility & Business Acumen: Ability to anticipate and respond to market shifts and translate external trends and organizational priorities into practical and scalable strategies, investments, and operating models. Understands the financial drivers of care progression, including capacity, length of stay, avoidable days, post-acute utilization, payer requirements, resource stewardship, and value-based performance.
• Change Management: Expertise in leading Ministry-wide transformation through governance, stakeholder alignment, adoption across diverse teams and settings, effective execution, communication, and performance measurement.
• Workforce Design: Ability to apply human-centered, experience-based design and workforce-development principles to improve patient, family, caregiver, and Mission Partner experiences.
