About The Position

Agrace, a nationally recognized nonprofit leader in hospice and palliative care, is seeking an experienced executive to lead our hospice homecare business through its next phase of growth and innovation. The ideal candidate brings a strong record of successfully leading complex healthcare organizations while remaining deeply committed to exceptional patient and family-centered care.

Requirements

  • BSN, with MBA or Master’s in Healthcare Administration or MSW required.
  • Minimum of 5 years prior leadership experience in a healthcare business, including experience working as part of a cross-functional executive leadership team, developing organizational culture, strategy and policy.
  • Minimum of 10 years of relevant leadership and progressive supervisory experience in an administrative position, including full accountability for both quality and financial outcomes, preferably in a hospice, home health or related program.
  • Demonstrated successful performance leading the clinical services of a healthcare entity with integrity, compassion, collaboration, excellence and stewardship.
  • Demonstrated experience in proactively seeking opportunities to monitor and improve patient and family care outcomes.
  • Demonstrated success in developing collaborative partnerships, including collaboration with competitors.
  • Demonstrated experience in quality, compliance and establishment of reporting and assessment mechanisms for continual review and improvement of all aspects of clinical operations, including regulatory compliance, quality and operational efficiency, in a healthcare environment.
  • Knowledge and experience with health care delivery systems, policies, accreditation standards, laws, regulations and reimbursement practices required; hospice-specific knowledge and experience preferred.
  • Ability to demonstrate exceptional theoretical and practical knowledge of clinical best practices.
  • Experience conceiving, designing, developing and executing strategic plans, business plans, project plans and administrative policies and procedures.
  • Ability to work with diverse patient populations with emphasis in home care, palliative care, and end of life issues.
  • Ability to understand and adhere to Hospice State License regulations, Medicare regulations and Community Health Accreditation Program (CHAP) standards.
  • Ability to build relationships with and work effectively with Boards of Directors, leadership, regulators and surveyors, health care providers, researchers, staff, volunteers, patients and families, and the general public.
  • Ability both to lead the clinical operation and to be a leader throughout the business; to model the mission and vision, create an environment of integrity and high trust, attract and retain the best talent, engage in healthy debate, challenge the assumptions of others and self, be a teacher and a learner and inspire and motivate others to achieve high goals.
  • Ability to conduct research in best practices, facilitate dialogue and decision-making, set strategy and direction, lead change, develop, implement and track initiatives and maintain accountability.
  • Ability to demonstrate polished and effective written and verbal communication skills, including group presentation skills and the ability to relate to varied audiences within and outside the agency.
  • Ability to think creatively and develop recommendations to improve the effectiveness and efficiency of processes and systems.
  • Ability to function effectively in a fast-paced and changing environment with multiple priorities and objectives; to operate with a sense of urgency while maintaining flexibility and effectively manage priorities and meet deadlines.
  • Valid Wisconsin driver’s license and car insurance.
  • Must have access to an automobile in good operating condition.

Responsibilities

  • Provide executive leadership for Agrace's home-based hospice operations with full accountability for performance, growth, access, and operational excellence.
  • Lead existing hospice homecare services while identifying and executing opportunities for expansion into new markets and geographies.
  • Design, implement, and optimize centralized and decentralized admissions and access models to ensure timely, equitable access to care.
  • Ensure delivery of consistent, high-quality clinical services that reflect Agrace's mission and values as a nonprofit healthcare organization, while maintaining strong financial and operational performance.
  • As a member of the executive leadership team, help shape and execute organizational strategy to expand access, strengthen care delivery systems, enhance partner relationships, and improve outcomes for patients and families.
  • Leads all hospice homecare operations across all markets.
  • Ensures consistent delivery of high-quality patient care aligned with regulatory standards and clinical best practices.
  • Standardizes clinical and operational practices to support growth across multiple markets, while allowing appropriate local adaptation.
  • Ensures care delivery models are scalable and sustainable as the organization expands.
  • Monitors quality indicators and trends and take corrective action when performance falls short.
  • Builds and develops a strong clinical leadership team, with clear succession plans.
  • Ensures effective staffing models that anticipate growth and changes in demand.
  • Collaborates with Medical Services, Compliance, and Regional Sales managers to ensure seamless care delivery.
  • Executes strategy for expansion into new markets, geographies, and service lines.
  • Develops scalable operating models that support growth while maintaining quality and consistency.
  • Oversees the design, integration, and performance of centralized and decentralized components of admissions and access functions.
  • Ensures timely, equitable access to care across all markets, with accountability for referral management, intake, and admissions outcomes.
  • Partners with Strategy, Finance, and Business Development to evaluate market opportunities, including volume projections, staffing models, and financial viability.
  • Aligns growth strategy with workforce capacity, regulatory requirements, and community needs.
  • Monitors growth and access metrics, takes corrective action when performance falls short.
  • Provides executive oversight of Access, ensuring alignment with clinical operations and growth strategy.
  • Defines clear roles, workflows, and accountability across centralized intake and local market teams.
  • Improves referral conversion, admission cycle time, and overall patient access experience.
  • Ensures smooth transition from referral through admission and initiation of care.
  • Aligns admissions performance with capacity planning and financial targets.
  • Serves as a senior leader shaping organizational strategy, priorities, and performance.
  • Leads design of systems that integrate clinical care, access, and growth across markets.
  • Drives alignment across centralized and decentralized functions to reduce variation and improve outcomes.
  • Holds leaders accountable for results across quality, growth, and access metrics.
  • Builds strong relationships with external partners to support growth and community presence.
  • Represents Agrace in local, state, and national forums related to hospice and end-of-life care.
  • Supports Board engagement by educating and informing on operational and clinical strategy.
  • Accountable for financial performance of hospice homecare services, including budget management and cost control.
  • Ensures alignment between growth strategy, staffing, and financial outcomes.
  • Monitors admissions performance and its impact on census and revenue.
  • Identifies performance gaps and takes corrective action to maintain financial health.
  • Maintains regulatory compliance and survey readiness across all services.
  • Supports organization-wide initiatives and strategic projects.
  • Completes required education and performs other duties as assigned.
