About The Position

Family Promise of the Big Bend is a nonprofit organization committed to preventing and ending homelessness among Veterans and their families through the VA Supportive Services for Veteran Families (SSVF) program. We deliver housing-focused, Veteran-centered services rooted in Housing First principles, collaboration, and accountability. The Veteran Outreach & Care Coordinator is the first point of contact for active clients and prospective veterans seeking support from our Veteran Services Program. This role conducts community outreach in assigned counties (outside of Leon County) to identify veterans in need and build partnerships with agencies, faith-based organizations, schools, and community groups. The Coordinator manages client engagement from intake through stabilization — including eligibility screening, referral to appropriate services, ongoing follow-up, and connection to housing and employment resources — while providing crisis de-escalation support and advocating for veteran needs. This is a hybrid position.

Requirements

  • Strong interpersonal, crisis de-escalation, and written and verbal communication skills.
  • Valid driver’s license, regular travel availability, and ability to pass an MVR check.
  • Knowledge of community resources and ability to foster working relationships with colleagues, clients, and community agencies.
  • High level of professionalism and ability to work independently.
  • Strong organizational, time management skills.
  • Working knowledge of Office suite and Adobe.
  • At least 3 years of experience working with veteran populations, homeless services, case coordination, outreach, or a related human services field.
  • Must be able to lift 25 lbs.

  • Master’s degree in Social Work preferred.

Responsibilities

  • Conduct weekly travel to assigned counties to conduct in-person outreach.
  • Serve as an ambassador for the organization at community events and meetings.
  • Conduct comprehensive intake and eligibility screening using standardized questionnaires and HMIS.
  • Work collaboratively with coworkers, supervisors, and community partners to ensure smooth enrollment and support provision.
  • Provide crisis intervention and case management related to housing stability.
  • Assess needs and barriers to meeting those needs.
  • Coordinate and support housing placement efforts.
  • Provide in-home visits for initial intake and follow-ups.
  • Develop individualized Housing Stability Plans.
  • Develop and maintain cooperative working relationships with community partners, such as the VA, CoC, THA, and others.
  • Request funding to assist in veterans’ housing stability.
  • Provide empathetic, trauma-informed de-escalation support for clients experiencing crisis over phone or in person and coordinate with other agencies.
  • Maintain accurate and timely client records, logs, HMIS, and referral documentation in accordance with organizational policies and funder requirements.
  • Provide updates and reports to supervisors to support program performance measurement.
