About The Position

Serves as the primary point of contact for veterans, active-duty service members, and their families within the CCBHC. Helps ensure engagement in the veteran communities/organizations, increase access to care, coordinates linkage and referral to behavioral health, medical, community, and VA resources, and assists veterans in navigating available benefits and supports. Promotes culturally competent, trauma-informed services consistent with CCBHC requirements, West Virginia behavioral health rules, and federal guidelines.

Requirements

  • High School Diploma or GED required.
  • Experience working with veterans required.
  • Valid driver’s license and vehicle required.
  • Must pass a Criminal Background Check and Employment Fitness Determination through WVCARES and a drug screen.
  • Knowledge of VA systems, military and veteran culture and veteran benefit programs and community resources.
  • Ability to develop rapport and effectively engage veterans and families.
  • Able to represent Seneca in a positive manner i.e. maintaining a positive attitude and performing duties in a manner in accordance with Seneca’s Code of Conduct & Ethical Practices.
  • Demonstrated knowledge of basic computer skills required

Nice To Haves

  • Bachelor's Degree in a Human Service field preferred.
  • Prior service in the US Armed Forces strongly preferred.

Responsibilities

  • Conduct outreach to veterans, service members, and military families in the community, including collaboration with local VA facilities, Vet Centers, community organizations, National Guard units, and veteran service agencies.
  • Increase awareness of CCBHC services through outreach events, presentations, and community partnership development.
  • Serve as a veteran-informed point of contact to reduce barriers to treatment and encourage early engagement in behavioral health and SUD services.
  • Assist veterans and families with connections to behavioral health services, crisis services, case management, peer support, and primary care screening.
  • Coordinate referrals to VA services (healthcare, benefits, housing, vocational services) in compliance with HIPAA and 42 CFR Part 2 when applicable.
  • Provide assistance with navigating systems such as TRICARE, VA benefit applications, CHAMPVA, housing programs, and community resources.
  • Arrange and/or assist with transportation linkage when needed; maintain documentation of all service coordination activities.
  • Follow up to ensure services arranged were accessed and assist with problem-solving barriers.
  • Provide non-clinical support and guidance to veterans experiencing behavioral health concerns, PTSD, SUD, homelessness, or reintegration challenges.
  • Offer supportive counseling within scope (non-therapeutic) and connect individuals to licensed professionals as appropriate.
  • Develop and maintain strong partnerships with federal, state, and local veteran-focused agencies and service providers.
  • Advocate internally for the unique needs of veterans to enhance culturally and trauma-informed care.
  • Participate in CCBHC interdisciplinary team meetings to support coordinated, team-based care.
  • Maintain complete, timely, and accurate documentation of all contacts, referrals, linkage efforts.
  • Track outreach statistics, access data, and service coordination outcomes for CCBHC reporting requirements.
  • Maintain veteran-informed and trauma-informed practices in all interactions.
  • Participate in team meetings.
  • Work closely with other team members.
  • Assure quality service for clients by becoming knowledgeable of and adhering to rules, regulations, and legal requirements and by documenting services provided in accordance with established standards.
  • Secure information by adhering to HIPAA and 42 CFR Part 2 regulations and Seneca privacy policies and keeping client information confidential.
  • Represent Seneca in a positive manner including maintaining a positive attitude and performing duties in a manner in accordance with Seneca’s Code of Conduct and Ethical Practice.
  • Other duties as assigned by supervisor.
