•$103,000 - $130,000

About The Position

The Utilization Management Lead, Inpatient Case Management, leads Akido’s inpatient case management function across inpatient status determination, transitions of care, and complex case management. This hands-on role reviews delegated inpatient and skilled nursing facility (SNF) admissions, manages post-discharge transitions, and carries a complex case management caseload for high-risk members, including direct outreach to members and caregivers. As the program scales beyond its current MVP, the Lead refines program standards and workflows and helps build the team structure needed to support membership growth. It works closely with the Medical Director, primary care providers, facilities, health plan partners, and operational teams. The Lead performs initial and concurrent reviews for delegated inpatient and SNF admissions using MCG and health plan criteria; coordinates with facility case managers on continued stay, discharge readiness, and post-acute placement; and documents reviews and determinations in EZCap. The role owns post-discharge follow-up, including notification response, member outreach, medication reconciliation, and coordination of follow-up visits. It also develops and maintains complex case management processes, care plans, policies, audit readiness, reporting, and compliance with applicable CMS, DMHC, and health plan delegation standards. The Lead reports program performance and recommends workflow changes, and coaches, trains, and onboards case management staff as the program grows. The position offers a salary range of $103,000–$130,000 USD.
