About The Position

The Urgent Care Case Manager provides support for clients who are at or near a point of crisis and needing additional support to stabilize. This may include clients experiencing first or early episode psychosis or other extreme states, clients who have discharged from a crisis stabilization program without established services and clients who are frequent contacts at the University of Vermont Medical Center Emergency Department and/or First Call for Chittenden County. Services are intended to be intensive and flexible in nature to manage the acuity of the situation including assist the client and/or family to find or maintain stability and connect to other services and resources, provide brief treatment, and coordinate care among providers or systems. This position is mobile throughout Chittenden County and coordinates with multiple stakeholders both in and outside of Howard Center to best meet client needs.

Requirements

  • Combination of education and relevant experience
  • Valid driver's license and use of a personal vehicle
  • Experience with assessment, diagnosis, crisis intervention and de-escalation
  • Vermont regulation mandates that if you do not hold a valid license then you must be on the Roster of Nonlicensed & Noncertified Psychotherapists before you can be hired for work in this position. If you are not yet on the Roster, please begin the process immediately to avoid delay in hiring. Note this process may take 3-5 days for in-state applications and up to several weeks for out-of-state applications. Instructions can be found on the website for the Vermont Office of Professional Regulation.

Responsibilities

  • Establish professional relationships with clients and families assigned to caseload
  • Provide urgent care case management/care coordination to children, youth, adults and families regardless of diagnosis
  • Provide brief treatment (crisis stabilization)
  • Transport clients in personal vehicle in the community when appropriate
  • Determine appropriate services/referrals a client needs
  • Assess ongoing needs of client and family and make referrals as appropriate
