About The Position

The WellBe care model is a physician-led, Advanced Practice clinician-driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to care. To address these problems, we have elected to bring care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facilities, to assisted living, to palliative care, to end of life care. Our WellBe physician/advanced practicing clinician-led geriatric care teams partner with the patient’s primary care physician to provide concierge-level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum. The Urgent Care Advanced Practice Provider (APP) is responsible for modeling the WellBe values and mission and will play a vital role in delivering white-glove, high-quality urgent care services to patients who need your help in the comfort of their own home. This position requires a combination of conducting in-person patient visits, supporting administrative tasks, and facilitating telehealth patient consults with APPs or Physicians as required. The Urgent Care APP role will serve as the local market expert and will act as a connector for patient support services across the WellBe platform. This role will be responsible for working closely with our clinical teams to deliver the highest quality of care to our patients. The role has no direct reporting responsibility but will function as part of an integrated, interdisciplinary patient care team. Urgent Care APP staff in these roles will help foster positive and effective working relations with senior leadership and management, local market management, and business ancillary departments for the purpose of achieving the company's vision, mission, and objectives. S/he is responsible for following the rules and regulations of state and federal regulatory agencies and other certified agencies for their region. Daily local travel to meet patients and the community will be an essential part of your role. Rotation through weekend coverage may be required.

Requirements

  • Minimum of Master's Degree, Nursing with focus on Family Nurse Practitioner, Adult Nurse Practitioner, Geriatric Nurse Practitioner or Adult-Geriatric Nurse Practitioner from an accredited, Nurse Practitioner program (required)
  • Active, unencumbered Nurse Practitioner license in the state of job posting and must reside in state (required)
  • Current board certification in your field with either ANCC (American Nurses Credentialing Center) or AANP (American Association of Nurse Practitioners) (required)
  • BLS certified and an active NPI# as an NP (required)
  • Active, unencumbered DEA licensure or eligibility to obtain (required)
  • Possess a valid driver's license with an acceptable driving record.
  • Requires proof of an active auto insurance policy.
  • Ability to conduct in-home setting visits and have previous experience on a telehealth platform
  • Demonstrated role proficiency with Microsoft Office; Excel, Word, G-suite, etc.
  • Proficient with Microsoft 365 software suite, Electronic Health Records, Scheduling Software, and Ring Central applications
  • Role model in courageous teamwork, integrity, ethics, and productivity
  • Excellent verbal, written, presentation, and interpersonal communication skills
  • Strong communication skills, with the ability to effectively interact with patients, families, and remote healthcare professionals.

  • Experience in palliative care, and end-of-life communication
  • Excellent verbal, written, presentation, and interpersonal communication skills

Nice To Haves

  • 3-5 years Nurse Practitioner experience (preferred)
  • Previous experience in value-based care, home care, primary care, hospice, palliative care, or geriatrics strongly preferred, other complex patient health care experience also beneficial (preferred)
  • Experience with telehealth care delivery (preferred)
  • Home care experience preferred.

Responsibilities

  • Practices the WellBe mission: To help our patients lead healthier, meaningful lives by delivering the most CompleteCare™ to their patients.
  • Will primarily support in-home, patient-facing urgent visits to support patient care needs in the market.
  • Facilitate in-home + assisted telehealth visits between WellBe clinical teams and patients, including urgent patient assessments and ongoing follow-up visits as appropriate per patient
  • Complete and document a thorough home safety inspection during patient visits.
  • Check and record health vitals, including weight, blood pressure, heart rate, lung sounds, and blood sugar levels, as well as potential health challenges noted during the home visit and/or reported by each patient.
  • Provides support for patient care needs with the support of the WellBe clinical team.
  • Updates patient records and checks records for accuracy at each patient
  • Responsible for patient education needs with the support of the WellBe clinical team.
  • Delivers the highest standard of quality care while maintaining all company metrics pertaining to appropriate patient needs.
  • Reviews patient care interventions and assessment of patient needs for transitions of care, end-of-life discussions, addressing HEDIS gaps (screening and med adherence), and palliative care interventions.
  • Completes and submits all documentation/paperwork requirements daily.
  • Attends Interdisciplinary Team (IDT) meetings and provides additional information about any patient or family interactions.
  • Collaboration with other care team members on patient care strategies.
  • Collaborates with the interdisciplinary team and participates in regularly scheduled team meetings.
  • Attends all required training, including medication, equipment/device training. Ongoing training is expected, and additional education, skills, and protocols will be added.
  • Build relationships with local community providers and caregivers to maintain WellBe's presence and reputation in local markets.
  • Utilizes all available resources to help care for patient needs and refers to internal/external resources as appropriate, such as WellBe behavioral health protocols.
  • Maintains required unencumbered, active state licensure(s).
  • Adheres to a professional code of conduct in patient care delivery, including completion of care needs and documentation standards.
  • Other tasks needed to accomplish organizational objectives/goals.
