About The Position

Sun River Health is seeking a full-time Unit Supervisor of Health Home Care Management to join their team in Amityville, NY. This role is responsible for the supervision of care management staff and the daily operations of the Health Home Care Management program. Key responsibilities include direct supervision of staff, ensuring the quality of care management services, managing program compliance, and conducting staff training. The Unit Supervisor will also ensure adequate support is provided to clients, particularly those with complex needs or in crisis.

Responsibilities

Ensure coverage and provide care coordination for assigned care manager caseloads and/or cover caseloads as needed
Provide crisis intervention to clients in psychological distress; review and approve client documentation as assigned.
Review assessments, plans, progress notes and other documentation as required to ensure necessary detail, content to meet regulatory requirements and standards
Develop and Implement quality assurance and quality improvement activities
Consult with multidisciplinary team on client’s care plan/needs/goals. This may include participation in case conferencing. May also include family members and social supports.
Provide supervision and support to staff assigned, including but not limited to assisting in hiring, scheduling, evaluating, ongoing staff development, team meetings and individual supervisory meetings
