Requirements

  • Current NYS RN license and registration
  • Associates degree required
  • Bilingual BSN new graduates encouraged to apply
  • A car and driver license is required for all regions except Manhattan.

Nice To Haves

  • Bachelor of Nursing degree preferred
  • One year of experience in Homecare, acute care or community health nursing preferred.
  • Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
  • Bilingual Spanish, Mandarin, Cantonese, Korean, Creole, Bengali, Hindi, and Arabic desirable

Responsibilities

  • Independently perform home visits to conduct functional, psychosocial and environmental safety assessments.
  • Determine eligibility of enrollees.
  • Establish an individual plan of care that enables them to remain safely at home despite long-term care needs.
  • Listen and communicate with a culturally diverse population that includes care givers as well as other providers.
  • Manage time effectively.
