Requirements
- Current NYS RN license and registration
- Associates degree required
- Bilingual BSN new graduates encouraged to apply
- A car and driver license is required for all regions except Manhattan.
Nice To Haves
- Bachelor of Nursing degree preferred
- One year of experience in Homecare, acute care or community health nursing preferred.
- Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
- Bilingual Spanish, Mandarin, Cantonese, Korean, Creole, Bengali, Hindi, and Arabic desirable
Responsibilities
- Independently perform home visits to conduct functional, psychosocial and environmental safety assessments.
- Determine eligibility of enrollees.
- Establish an individual plan of care that enables them to remain safely at home despite long-term care needs.
- Listen and communicate with a culturally diverse population that includes care givers as well as other providers.
- Manage time effectively.
