About The Position

Provides federally regulated Medicaid case management services under Title XIX programs in compliance with federal Medicaid rules, Washington Administrative Code (WAC), and agency policy. Responsible for conducting comprehensive CARE assessments, authorizing long-term care services, issuing Planned Action Notices (PANs), and ensuring regulatory compliance. Responsible for assessment of client needs, development and monitoring of service plans, makes appropriate referrals and serves as a customer advocate with other service providers. Assists with department day-to-day operations as delegated, including reports, outreach, community events, support groups, and more. Requires knowledge of related community resources and the laws and regulations specific to the area of service.

Requirements

  • Four years of paid, full-time, on-the-job social service experience in providing care management and long-term support and services to vulnerable adults, such as providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads, and a high school diploma or its equivalent; or
  • Three years of paid, full-time, on-the-job social service experience, providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads, a high school diploma or its equivalent, and are bilingual or bicultural with such experience; or
  • Two years of experience with other DSHS information systems used by Case Management staff, such as Comprehensive Assessment and Reporting Evaluation (CARE) tool, Barcode, ProviderOne, QA Monitor, ACES.Online, or ALTSA reporting, and college courses, an Associate’s degree, or its equivalent; or
  • Two years of paid, full-time, on-the-job social service experience and Bachelor’s degree; or
  • One year of paid, full-time, on-the-job social service experience and a Bachelor’s degree in social services, behavioral or health sciences; or
  • Social service experience and a Master’s degree.
  • One year full-time equivalent verifiable hours through an internship or volunteer work in social services may be substituted for one year of paid social service experience in providing care management and long-term support and services to vulnerable populations. Part-time hours may be considered if the hours total one year full-time equivalent.
  • Individuals functioning as Case Managers prior to August 28, 1991, who have performed competently as determined by their performance evaluations, may be deemed Case Managers from a Washington State Area Agency on Aging when they meet the following educational requirements: High School diploma or its equivalent and four years of paid on-the-job social service experience prior to August 28, 1991; and Completion of state-sponsored CORE training.
  • Valid driver’s license in state of residence.
  • Auto insurance in the amount required by the State of Washington.
  • Successful completion of a criminal history background check.
  • To support operational needs and compliance requirements, this position is expected to be performed within the State of Washington.

Responsibilities

  • Conduct comprehensive CARE assessments.
  • Authorize long-term care services.
  • Issue Planned Action Notices (PANs).
  • Ensure regulatory compliance.
  • Assess client needs.
  • Develop and monitor service plans.
  • Make appropriate referrals.
  • Serve as a customer advocate with other service providers.
  • Assist with department day-to-day operations as delegated, including reports, outreach, community events, support groups, and more.
  • Complete initial interviews with clients and their families to assess customer eligibility for services.
  • Gather relevant and useful data from the client, family, and other agencies to formulate a psychosocial assessment of the client and their family.
  • Assemble and guide group discussions and decision-making sessions among relevant professionals and program representatives, clients and their families, and significant others to formulate goals and design an integrated intervention plan.
  • Act as a liaison between the client and their family and all relevant professionals, programs, and informal resources involved in the overall intervention plan to help the client make their preferences known and secure the services needed.
  • Provide ongoing emotional support to the client and their family so they can cope better with problems and utilize professional and complex services.
