About The Position

The Supervised Independent Living (SIL) Case Manager provides support and direction for youth. Develops an individualized life skills program for each youth. Demonstrates and teaches skills that are necessary for youth to live independently. Locates appropriate community resources and establishes links. Advocates on behalf of youth. Implements service plans relative to emancipation. Coordinates ancillary services with the youth. Acts as a resource to Tabor Children’s Services’ (TCS) staff regarding adolescent and independent living issues.

Requirements

  • Bachelor’s degree in Social Work or related field
  • Two years of experience working within child, youth, and family service systems.
  • Valid driver’s license and the ability to use your own vehicle.
  • Clean driving history record.
  • Must be free from any communicable or infectious diseases.
  • Provide a valid PA child abuse clearance, FBI check, and criminal background check.
  • Knowledge of PA Code 55 3680s, 3700s, and the Philadelphia DHS scopes of service.

Responsibilities

  • Maintains a caseload of independent living youth.
  • Contacts with each youth as mandated by SIL standards and regulations.
  • Develops and implements an individualized service plan that includes life skills building and discharge planning for all youth in compliance with standards and regulations.
  • Develops all resources necessary for service provision, including the location of the apartment.
  • Works with biological families, when appropriate, to facilitate the goals of an individual service plan.
  • Coordinates all systems and services involved with youth.
  • Collaborates with TCS staff, the referring agency, and all other professionals.
  • Develops group structure to present life skills and support to all older adolescents.
  • Maintains a complete client file, including an ongoing written record of all client and collateral contracts, mandated reports and plans, and any other written work necessary for regulation compliance.
  • Develops and implements a discharge plan for youth.
  • Assess youth and their relationships with biological family and peers, taking into consideration the educational, emotional, psychological, and physical needs of the client.
  • Assess biological family members, their relationship to youth, and their ability to provide support during and after independent living.
  • Complete the required assessments according to timelines.
  • Provides advocacy for youth.
  • Provides crisis intervention and resolution as needed with individual caseloads.
  • Other duties as assigned.
