POSITION OVERVIEW: Title: Tribal Registered Nurse (Director Level Position) Reports To: Chief Medical Officer Schedule: M-F; 8am-5pm Status: Salary - Exempt Summary: The purpose of this position is to provide quality, comprehensive nursing services to Tribal individuals and families as well as provide oversight to our Tuolumne and Sonora Medical Clinics through comprehensive management of nursing related services. This role focuses on the prevention of illness and promotion and maintenance of health of high-risk tribal members through comprehensive nursing services, including but not limited to outreach, follow-up, accompanying on or scheduling visits, coordinating care and coordinating medication management. The Tribal RN shall provide or coordinate therapeutic services, home visits, education, and advocacy services through the assessment and identification of the individual, family, and community needs. establishing health goals, planning programs that meet identified needs and coordinating health programs and services. The Tribal RN is also responsible for the supervision of the Tribal Member Well-Being Department in a manner that is consistent with the philosophy and objectives of TMWIHC. This individual will ensure that systems are in place to safeguard patients' Medi-Cal information in accordance with HIPAA regulations and requirements.

Requirements

  • Graduate of an accredited school of nursing and Current Registered Nurse (RN)
  • Current Provider Basic Life Support (BLS) card
  • 2-years of Manager level experience in a Clinical setting or demonstration of equivalent experience and education
  • knowledgeable of chronic conditions
  • The ability to observe and react to adverse health indicators exhibited by patients and have the ability to respond appropriately in emergency situations.
  • The ability to effectively relate to Tribal members, gaining their trust and confidence.
  • The ability to plan, assign and direct the work of others.
  • The ability to communicate effectively orally and in writing; and the ability to follow oral and written instructions.
  • Must possess sufficient written and verbal communication skills to be able to interact with co-workers, employees, patients, Medi-Cal professionals, vendors, and agencies in a professional manner. In order to develop rapport and enhance business relationships.
  • The ability to work in an independent and self-directed manner and display individual initiative.
  • Must be willing and have the ability to accept supervision and to follow directions.
  • Must possess skill in using computers and other office equipment; in gathering and summarizing information; preparing reports; and in planning, organizing, prioritizing, and executing complicated and continuing assignments.
  • Must have experience and ability to use Electronic Health Records.
  • Must have the ability to maintain confidentiality and handle sensitive matters with discretion and tact.
  • Must have a general knowledge of and the ability to maintain HIPAA privacy requirements.
  • Must have the ability to work in a fast-paced and sometimes stressful environment.
  • Must be willing and able to travel as needed.
  • Must pass a comprehensive background investigation and be free of any felony convictions.
  • Must Maintain a professional appearance and attitude.
  • Must have a vehicle, valid California driver's license, a clean driving record, and personal automobile insurance with $100,000/$300,000 liability limits and $100,000 property damage limits.
  • Must be insurable by TMWIHC.

Responsibilities

  • Acts as the secondary Infection Control Lead for all clinical locations
  • Annually, review and/or revise Tribal Member Well-Being Department policies, procedures, objectives, and work plans.
  • In conjunction with the Chief Medi-Cal Director and/or Director of Nursing, develop short- and long-range plans for improving community health standards. Such plans will incorporate evaluating tools designed to assess current community health standards.
  • Develop and implement staff in-service training on a regularly scheduled basis.
  • Develop and submit proposals for funding of Tribal Member Well-Being Department objectives.
  • Coordinate the collection and tabulation of data, which represents all monthly activities of staff.
  • Establish and maintain a tracking system to ensure timely and appropriate action on referrals to and from the Tribal Member Well-Being department.
  • Oversee LVN Clinic Nurse Manager and Clinic Service Manager
  • Assist in providing orientation and training for all new Tribal Member Well-Being staff.
  • Oversee Enhanced Care Management Coordinator, Community Health Worker and other employees as needed to meet objectives of Tribal care.
  • Be familiar with the aims and objectives of Health and Well-Being for our Tribal community and express genuine enthusiasm for their success.
  • Perform or coordinate in home skilled nursing procedures and treatments while teaching qualified Native American individuals and families to care for themselves.
  • Utilize the nursing process of assessment, nursing diagnosis, planning, intervention and evaluation to ensure the plan of care consistently reflects current health conditions, interventions, expected outcomes, appropriateness of the level of care, and frequency of home visits.
  • Identify and track all hospitalized qualified Native American patients.
  • Provide follow-up home care and health education as necessary.
  • Receive TMWIHC interdepartmental and outside health agency referrals for the purpose of providing home visits to acute and chronic care patients.
  • Provide health education services to qualified Native American individuals and groups, with emphasis on Chronic Disease Management. Such services include but are not limited to education regarding diabetes and hypertension prevention and management, women's and men's health, substance abuse and communicable disease control.
  • Maintain an immunization program for all registered TMWIHC Native American Indian patients in compliance with state, federal and IHS guidelines.
  • Contribute program notes and care plan/updates to Medi-Cal record for each client served by Outreach staff.
  • Assure charting complies with IHS regulations or guidelines.
  • Maintain a client high-risk list for each area.
  • Coordinate care with patients and their TMWIHC primary care providers.
  • Work hand in hand with Elder Home Care and Elder program staff
  • Other duties as assigned.
