About The Position

This is a full-time, benefits-eligible position for a Triage Specialist RN with Pediatric and Adolescent Practitioners. The role operates Monday through Friday from 8 am to 5 pm in Gahanna, OH. The essential functions include conducting telephone triage for patient health complaints using an electronic triage assessment tool (Schmitt-Thompson), navigating multiple electronic platforms to assess health complaints, patient charts, and scheduling, and ensuring appropriate and timely documentation. The role also involves documenting phone call details for data tracking, assessing and closing patient care gaps, and adhering to all policies and procedures. A key aspect of the role is developing positive and effective working relationships with COPC staff, offices, and physicians.

Requirements

  • Active and unrestricted registered nurse license in the state of Ohio
  • Two years minimum Registered Nurse experience in a hospital and/or medical practice setting
  • Exceptional active listening, communication, and documentation skills
  • Excellent customer service skills, and mind-set
  • Ability to collaborate with a team
  • Ability to adapt to department growth, expansion, and policies/ procedures
  • Experience in pediatrics required
  • Basic knowledge of Microsoft Office Programs, such as Excel
  • Proficient knowledge of electronic health records

Nice To Haves

  • Experience in telephonic nursing triage

Responsibilities

  • Conduct telephone triage for patient health complaints using an electronic triage assessment tool (Schmitt-Thompson)
  • Navigate multiple electronic platforms to assess current health complaint, patients’ electronic chart, PCP appointment scheduling, and ancillary center scheduling
  • Ensure appropriate and timely documentation
  • Document phone call details in tracking system for department data tracking and collection
  • Assess, identify, and close patient care gaps
  • Follow and execute all policies, procedures, and job responsibilities instructed by direct supervisor
  • Develop positive and effective working relationship with COPC staff, offices, and physicians
