About The Position

VNA Hospice NWI is hiring a full-time Triage RN for Home Hospice. This role involves providing professional nursing support via phone to patients, families, and callers, dispatching on-call staff for in-person support, and coordinating after-hours service delivery. The triage nurse will also provide initial bereavement support, document all interactions, and act as the first line of support outside of regular business hours, initiating the intake process as needed. This position supports field staff and may assist with complaints. The coverage area includes Porter, LaPorte, Starke, Jasper, Newton, and Lake County, with mileage paid for visits.

Requirements

  • Must have and maintain license as a registered nurse by the state of Indiana.
  • Minimum of associate degree in nursing, graduate of a school of professional nursing.
  • Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home care.
  • Must be sensitive to issues of loss and grief.
  • Must maintain current CPR certification.
  • Must hold and maintain a valid Indiana driver’s license and auto insurance.

Nice To Haves

  • Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred.

Responsibilities

  • Support patients, families, and callers via phone with professional nursing support.
  • Dispatch on-call staff and other hospice care team members for in-person support.
  • Coordinate and prioritize all service delivery after hours, including patient and family education.
  • Dispatch interdisciplinary visits and coordinate services with external vendors and resources.
  • Provide initial bereavement support to families.
  • Document all interactions with patients, families, contacts, and vendors in the patient record.
  • Promote a positive customer service-oriented approach.
  • Serve as the first line of support for assistance needed outside of regular business hours.
  • Initiate the intake process as needed.
  • Support staff members and work in the field as needed.
  • Initiate the complaint process as needed per organization’s policy.
