About The Position

Full-Time Triage RN - Community Hospice- Eves and Overnights. This role involves a mix of evening and overnight shifts during the week, with rotating weekends from 8a-8p. The Triage RN will serve as a vital resource for patients, families, and caregivers by responding to symptom management calls during evenings, overnights, and weekends. Working closely with the on-call hospice team, the RN will provide clinical guidance, emotional support, and care coordination. This is a remote position, but the employee must live locally for training and potential in-office needs.

Requirements

• Current license to practice as a Registered Nurse in the State of New York
• Associates Degree in Nursing, BSN preferred
• Minimum of 1 year of RN experience required
• Strong assessment, communication, and critical thinking skills
• Ability to actively listen and respond appropriately to patients/families
• Ability to support patients/families through times of crisis.

Nice To Haves

• Home care and Hospice experience preferred

Responsibilities

• Respond to phone calls from patients and/or caregivers.
