About The Position
Beyond Hospice is seeking an experienced and compassionate Registered Nurse (RN) – After-Hours Triage to support hospice patients and families during evening and overnight hours. This role offers independence, meaningful hospice work, and the opportunity for extended time off. The RN will provide telephone triage, make patient visits when clinically necessary, and offer comfort-focused nursing care and symptom management.
Requirements
- Graduate of an accredited school of nursing.
- Current Minnesota RN license.
- Excellent observation, verbal and written communication, problem solving skills; nursing skills per competency checklist.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- Ability to lift, push, or pull up to 50 lbs.
- Frequent standing, walking, bending, and reaching.
- Ability to assist patients with mobility and transfers as needed.
- After-hours and weekend availability as needed.
Nice To Haves
- Hospice, home health, acute care, or triage experience preferred
- Bachelor’s degree preferred.
- Two (2) years of acute care experience in an institutional setting, or two (2) years of recent experience in hospice/home care.
Responsibilities
- Provide after-hours telephone triage for hospice patients and families
- Assess symptoms and determine appropriate interventions
- Make in-person visits when needed
- Provide comfort-focused nursing care and symptom management
- Educate and support patients, families, and caregivers
- Collaborate with physicians and the hospice interdisciplinary team
- Complete timely and accurate clinical documentation
