The Triage Nurse is responsible for reviewing patient histories and documents to make accurate assessments, identifying and documenting patient problems, and assessing nursing home patient history. This role involves checking insurance, medications, and Advance Directives, as well as obtaining medical device cards. The Triage Nurse assigns priority to patient concerns, initiates diagnostic and therapeutic measures, and acts as a liaison between patients, families, and health professionals. They coordinate patient needs with supporting departments, chart patient progress, manage prescription refills, and follow directions on the Recall system. Additionally, the Triage Nurse calls for test results, notifies patients, sets up lab appointments, provides patient education, follows disease process protocols, and follows up on patient resources, keeping the physician informed. They address patient phone inquiries, complete physician's orders, schedule testing at outside facilities, and maintain their knowledge base through professional education. The role also involves assisting with patient referrals and demonstrating respect for confidentiality. In terms of guest relations, the Triage Nurse demonstrates courteous and cooperative behavior, participates in Quality Assessment, cooperates with other departments, precepts new employees, assists in implementing unit changes, adapts to changes in assignments and workload, and cooperates with the Clinic Supervisor to ensure adequate staffing, keeping the supervisor informed of personnel issues.
