About The Position

The Triage Nurse Supervisor - RN will supervise and provide direction to after-hours nursing staff and on-call team members to ensure quality, compliance, assessment, and reassessment of patient needs and continuity of services. This role supports patient care managers at multiple sites to ensure continuity of staff and patient needs during after-hours periods. The supervisor will coordinate triage and after-hours staff schedules and logistics, serve as the point of escalation for complex patient situations, and provide clinical support and triage oversight.

Requirements

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current State license as a Registered Nurse
  • Current State Drivers License, reliable transportation, and valid and current auto insurance
  • Two years of experience as a Registered Nurse in a clinical care setting, hospice or home health
  • Demonstrated ability to supervise
  • Ability to establish and effective working relationships with all segments of the staff, the lay and professional public, the Board of Directors and Medical Director
  • Computer proficiency with MS Office products including Word and Outlook
  • Proof of current CPR

Nice To Haves

  • Telephonic Triage experience preferred
  • Experience in EMR a plus

Responsibilities

  • Supervise and provide direction to after-hours nursing staff and on call team members, in an effort to ensure quality, compliance, assessment and reassessment of patient's needs and continuity of services by appropriate health care personnel
  • Support patient care managers at multiple sites to provide continuity of staff and patient needs during after hours
  • Coordinate triage and after-hours staff schedules and logistics to maintain adequate staffing requirements for after hours, weekends, and holidays
  • Direct report for after-hours triage nurses, including scheduling, performance evaluation, and issue resolution
  • Serve as the point of escalation for complex patient situations
  • Provide clinical support and triage oversight to ensure accurate nursing assessments and triage decisions are being made
  • Ensure smooth communication between after-hours and daytime teams for continuity of care
  • Provide shift reports, incident logs, and other updates to daytime patient care managers or executive directors
  • Communicate and educate policy changes or updates to on call staff members
  • Onboard and train new triage nurses
  • Provide coaching, development, feedback to after-hours team members in a professional and timely manner
  • Ensure staff are trained and updated on new triage protocols and clinical guidelines
  • Timely submission of all documentation
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
