Answer triage calls during assigned shifts while maintaining a calm, reassuring presence during emotionally intense situations. Assessing patients, symptoms, pain levels, and other concerns over the phone to determine the urgency of care and the need for in-person visits. Schedule and manage overnight visits, ensuring appropriate staffing coverage and efficient routing Determine urgency of needs and coordinate dispatch of on-call nurses or other hospice team members as necessary. Providing guidance on managing hospice-related symptoms like pain, nausea, and difficulty breathing, recommending appropriate medication adjustments or interventions. Educating patients, families, and caregivers on the dying process, end-of-life care, and symptom management techniques. Accurately documenting all patient interactions, interventions, and follow-up actions in the electronic medical record. Provide guidance on managing pain, shortness of breath, anxiety, nausea, and other symptoms. Collaborate with on-call nurses, physicians, and other hospice team members. Providing after-hours coverage for patient needs, including phone triage and on-call visits as needed. Adhering to standards for infection control, sanitation, and patient safety and confidentiality This role may require working on call as needed to cover call offs, terminations, vacations, etc All other duties as assigned
