About The Position

Responsible for guiding clients through the healthcare process across the organization, working closely with the Outpatient Case Management Team. This role will educate clients on their healthcare options and ensure they have a clear understanding of their care process. Additionally, this position will assist with Monthly Progress Reports for clients with a referral source, requiring strong organizational skills and attention to detail.

Requirements

  • High school diploma or completion of G.E.D
  • At least one year of customer service experience

Nice To Haves

  • Associate’s degree or two-year college certificate in business or healthcare administration preferred
  • At least one year of healthcare related customer service experience
  • Prior data entry experience using MS Outlook, Word, Excel and an electronic medical record
  • Bilingual/Bi-cultural

Responsibilities

  • Provide exceptional customer service that is both professional and accommodating.
  • Provide support to clinicians and clients, both on and off site.
  • Track clients through services and support attending providers with outreach and engagement efforts.
  • Complete outreach calls to clients regarding services, answer questions regarding treatment options and requirements.
  • Assist with adding referrals to the group list or the case management referrals list.
  • Outreach to clients for group enrollment or placement with a case manager for management services.
  • Provide case management screening and linkage from referrals to be assigned to ongoing case management services.
  • Complete an initial chart audit of case management chart for compliance prior to assigning for ongoing case management services.
  • Send clients’ immediate needs in referrals, treatment signature page and ROI’s for coordination of services as needed.
  • Maintain electronic medical record charts.
  • Assist with ongoing chart audits.
  • Receive and screen telephone calls, prepare correspondence, route mail and perform other associated tasks of this nature.
  • Document all interactions in Electronic Health Record, at the front desk for certain letters and communication to be sent out to clients.
  • Perform other duties as assigned.
