Requirements

• Licensed Nurse Practitioner or Physician Assistant.
• At least two years of experience working with the elderly.
• Must have prescriptive authority.
• Must have DEA registration.
• Eligible to serve Medicare/Medi-Cal participants.
• CPR and First Aid certification upon hire preferred.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter.

Responsibilities

• Provide life-changing, community-based care to vulnerable seniors.
• Build deeper, long-term relationships with fewer patients.
• Provide comprehensive, holistic care within a value-based PACE program.
• Conduct home visits.
• Participate in a rotating on-call schedule (phone only).
• Travel throughout California clinics.
