About The Position

At WelbeHealth, participants are guided by an interdisciplinary team (IDT) of clinical and non-clinical members who work together to provide all-inclusive care to vulnerable seniors. The Traveling Physical Therapist coordinates and provides restorative and rehabilitative physical therapy services to participants at multiple day centers and in participants’ homes, as assigned. The role works closely with the PACE Physician, rehabilitation staff, and other IDT members to maximize participant independence and safety and to develop and restore function.

Requirements

• At least five years of progressive physical therapy experience.
• At least two years of physical therapy experience in a clinical setting with a frail or elderly population.
• Bachelor’s degree in physical therapy.
• Ability to work independently with minimal supervision.
• Valid California Physical Therapy License or proof of license eligibility.
• Obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter.

Nice To Haves

• Master’s degree in physical therapy.
• CPR and First Aid certification upon hire.

Responsibilities

• Conduct physical therapy assessments in facilities and participant homes to define participants’ care needs, preferences, and goals.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document physical therapy interventions in accordance with physician orders and/or participant care plans.
• Provide active and passive exercise, muscle re-education, gait and functional training for activities of daily living, and treatments such as massage, heat, sound, water, light, or electricity.
• Oversee Therapy Assistants and provide the training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP).
• Train and instruct staff, caregivers, participants, and families in the use and care of therapeutic equipment.
