Transition Home Healthcare is seeking motivated and skilled Physical Therapists (PT), Physical Therapist Assistants (PTA), Occupational Therapists (OT), Certified Occupational Therapy Assistants (COTA), and Speech Language Pathologists (SLP) to join our growing Travel Home Health Team! This is an exciting opportunity for therapists who enjoy autonomy, patient-centered care, and the flexibility of traveling throughout the Greater Houston area while earning highly competitive compensation and travel-related benefits.
Requirements
- Current Texas license in applicable discipline (PT, PTA, OT, COTA, or SLP)
- Strong clinical, communication, and organizational skills
- Reliable transportation and valid driver’s license
- Ability to work independently and adapt to varying patient needs
- Willingness to travel throughout assigned Houston territories
- Compassionate, dependable, and patient-focused mindset
Nice To Haves
- Home health experience preferred, but not required
Responsibilities
- Provide therapy evaluations and treatment services in patients’ homes
- Develop and implement individualized treatment plans
- Complete timely and accurate clinical documentation
- Collaborate with physicians, nurses, and interdisciplinary care teams
- Educate patients and caregivers regarding treatment goals and home exercise programs
- Monitor patient progress and adjust treatment plans as appropriate
- Maintain compliance with company policies, Medicare guidelines, and healthcare regulations
- Travel to assigned patient locations throughout the Greater Houston & surrounding areas
