About The Position

Jukebox Health is seeking an Occupational Therapist to help older adults and individuals with disabilities live safely and independently at home. The company partners with health plans to provide personalized home modifications and environmental supports, combining technology with networks of clinicians, suppliers, and installers. This role travels as needed to conduct in-home visits in expanding markets. The successful candidate is empathetic, comfortable balancing client, program, and business needs, and able to use new systems and tools. Prior home-modification experience is a plus.

The position is part-time, with guaranteed compensation of $45 per hour ($1,080 per week), paid every two weeks. Travel is managed and paid through RampTravel, with budgets for airfare, hotels, and food stipends. Use of the employee’s own automobile for travel is reimbursed at $0.70 per mile. The anticipated start date is February 1, 2026.

Requirements

• Degree in Occupational Therapy from an accredited program.
• At least 2 years of clinical experience.
• Strong interpersonal and clinical reasoning skills.
• Reliable transportation and a valid driver’s license.
• Proficiency with technology and digital documentation.
• Ability to work independently while collaborating with a remote team.

Nice To Haves

• Experience in home health, aging in place, or disability services.
• Past or current experience with home modification work.

Responsibilities

• Complete required onboarding and credentialing.
• Participate in ongoing training and development programming as needed.
• Conduct in-home assessments and functional evaluations.
• Provide personalized, evidence-based recommendations for home modifications.
• Educate clients and caregivers about fall prevention and safe mobility techniques.
• Engage clients and families in collaborative care planning.
• Maintain accurate and timely documentation in the electronic system.
• Support ad hoc clinical initiatives.
