About The Position

Hueman is hiring Travel Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members throughout the state, with potential to cover neighboring states as well (pending licensure). Requirements

• Master’s Degree (MSN) or higher from an accredited NP program
• Active Nurse Practitioner license (multistate licensure preferred)
• National Board Certification
• Current BLS or ACLS certification
• 12+ months of experience as a paid NP is required

Responsibilities

• Conduct comprehensive, non-invasive in-home health assessments
• Review medical history and perform medication reconciliation
• Identify care gaps and document findings
• Provide patient education on wellness and prevention
• Complete accurate, timely electronic documentation
