About The Position

Navitas Healthcare, LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a long-term care setting. This role involves delivering high-quality nursing care to residents, supervising support staff, and ensuring compliance with clinical and regulatory standards. The ideal candidate will demonstrate strong clinical judgment, leadership skills, and a commitment to resident-centered care.

Requirements

  • Graduate of an accredited nursing program or approved LPN program
  • Active, unrestricted LPN license in the state of Massachusetts
  • Current BLS / CPR / AED certification required
  • Must be able to read, write, and communicate effectively in English
  • Physically capable of lifting up to 50 lbs and meeting job demands

  • Prior experience in long-term care preferred
  • Strong knowledge of nursing practices, procedures, and regulatory standards
  • Ability to supervise staff and make sound clinical decisions independently
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively with residents, families, and healthcare teams

Responsibilities

  • Provide direct nursing care in accordance with established care plans and physician orders
  • Supervise and support nursing assistants in daily resident care activities
  • Administer medications and treatments safely while verifying patient identity
  • Monitor resident conditions and document changes accurately in medical records
  • Transcribe and implement physician orders with attention to detail
  • Assist with admissions, transfers, and discharges, including required documentation
  • Perform clinical procedures such as wound care, catheterization, tube feedings, and specimen collection
  • Measure and record vital signs (temperature, pulse, respiration, blood pressure)
  • Communicate effectively with physicians, residents, and family members regarding care plans
  • Collaborate with interdisciplinary teams in care planning and delivery
  • Maintain adequate supplies and report needs to the Nurse Supervisor
  • Ensure adherence to infection control protocols and patient confidentiality standards
  • Promote resident safety, comfort, and overall well-being
  • Participate in quality improvement initiatives, surveys, and inspections
