About The Position
Navitas Healthcare, LLC is seeking a dedicated LPN to provide high-quality care in a Long-Term Care (LTC) facility. The LPN will assist patients with daily living activities, administer medications, and support the healthcare team in maintaining a safe and healthy environment.
Requirements
- Licensed Practical Nurse (LPN) license
- Experience in Long Term Care (LTC) preferred
- Ability to administer medications and injections
- Proficiency in performing wound care and dressing changes
- Skill in collecting laboratory samples
- Strong documentation and record-keeping skills
- Ability to observe and report patient condition changes
- Excellent communication and interpersonal skills
- Ability to provide emotional support and education
- Commitment to maintaining a safe and organized patient environment

Responsibilities
- Monitor and record vital signs (blood pressure, temperature, pulse, respiration)
- Administer medications and injections as prescribed
- Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, and feeding
- Perform wound care and dressing changes
- Collect samples for laboratory testing
- Maintain accurate patient records and documentation
- Observe and report changes in patient condition to RN/physician
- Provide emotional support and education to patients and families
- Ensure a clean, safe, and organized patient environment
