Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking a Home Health Physical Therapist for a travel assignment in Medford, Oregon. This role supports a home health program delivering one-on-one patient care across a defined territory, focusing on rehabilitation, mobility, and independence in the home setting. POSITION OVERVIEW This assignment is ideal for experienced Home Health PTs who are confident working independently, managing their own caseload, and completing OASIS documentation accurately and efficiently. ASSIGNMENT DETAILS • 13-week contract • Day shift 5x8s (08:00–16:30) • 1:1 patient visits • Up to 6 visits per day (30 productivity points) • No weekends required • No call required • Local candidates accepted at same rate • 1 position available COMPENSATION Because travel rates change frequently based on market demand and facility needs, our recruiters provide the most current and competitive pay packages during your initial conversation. Each compensation package is customized and may include taxable hourly wages along with tax-free stipends when applicable. CLINICAL RESPONSIBILITIES • Perform home health PT evaluations and treatments • Complete OASIS assessments and documentation • Develop and implement individualized care plans • Provide home exercise programs and patient education • Treat orthopedic, neurologic, and post-surgical patients • Promote independence and functional mobility • Document all care in Epic/OASIS system WORK ENVIRONMENT • Home health setting (field-based) • Independent caseload management • Travel within assigned territory (up to 50+ mile radius) • Mileage reimbursement provided (IRS standard) Heartline Staffing is a Strategic Talent Partner with Purple Cow Recruiting. Requirements
- Minimum 1 year Skilled Home Health experience REQUIRED (HARD STOP)
- OASIS experience REQUIRED (HARD STOP)
- First-time travelers NOT accepted
- BLS REQUIRED (in hand at submission)
- Must have reliable transportation
- Must be comfortable working independently in the field
- OR PT license REQUIRED (pending accepted, must be active prior to start)
