Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking a Home Health Physical Therapist for a travel assignment in Medford, Oregon. This role supports a home health program delivering one-on-one patient care across a defined geographic area, focusing on functional mobility, rehabilitation, and patient independence. POSITION OVERVIEW This assignment is ideal for Physical Therapists with strong home health experience who are comfortable working independently, managing their own schedule, and providing care in a variety of patient home environments. ASSIGNMENT DETAILS • 13-week contract • Day shift 5x8s (08:00–16:30) • 1:1 patient visits • Up to 6 patients per day (30 productivity points standard) • No weekends required • No call required • Local candidates accepted • 1 position available COMPENSATION Because travel rates change frequently based on market demand and facility needs, our recruiters provide the most current and competitive pay packages during your initial conversation. Each compensation package is customized and may include taxable hourly wages along with tax-free stipends when applicable. CLINICAL RESPONSIBILITIES • Provide home-based physical therapy evaluations and treatments • Develop and implement individualized care plans • Perform OASIS documentation and assessments • Deliver patient and family education for continued care • Treat orthopedic, neurologic, and post-surgical conditions • Promote mobility, strength, and independence in home settings • Document all patient care in Epic/OASIS system WORK ENVIRONMENT • Home health setting (field-based) • Independent scheduling and patient visits • Travel within assigned territory (up to 50+ miles radius) • Mileage reimbursement provided (IRS standard) Heartline Staffing is a Strategic Talent Partner with Purple Cow Recruiting. Requirements
- Minimum 1 year Skilled Home Health experience REQUIRED (HARD STOP)
- OASIS experience REQUIRED (HARD STOP)
- First-time travelers NOT accepted
- BLS REQUIRED (in hand at submission)
- Must have reliable transportation
- Must be comfortable with independent field work
- OR PT license REQUIRED (pending accepted, must be active prior to start)
