Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking a Home Health Physical Therapist for a travel assignment in Medford, Oregon. This role supports a home health program delivering individualized patient care across a defined territory, focusing on rehabilitation, mobility, and functional independence in the home setting. POSITION OVERVIEW This assignment is ideal for Physical Therapists with strong home health experience who are comfortable working independently, managing their own caseload, and delivering care across a variety of home environments. ASSIGNMENT DETAILS • 13-week contract • Day shift 5x8s (08:00–16:30) • 1:1 patient visits • Up to 6 visits per day (30 productivity points standard) • No weekends required • No call required • Local candidates accepted • 1 position available COMPENSATION Because travel rates change frequently based on market demand and facility needs, our recruiters provide the most current and competitive pay packages during your initial conversation. Each compensation package is customized and may include taxable hourly wages along with tax-free stipends when applicable. CLINICAL RESPONSIBILITIES • Perform home health physical therapy evaluations and treatments • Complete OASIS assessments and documentation • Develop individualized care plans and home exercise programs • Provide patient and family education for continued care • Treat orthopedic, neurologic, and post-surgical conditions • Promote patient independence and functional mobility • Document all care in Epic/OASIS system WORK ENVIRONMENT • Home health (field-based care) • Independent scheduling and patient visits • Travel within assigned territory (up to 50+ mile radius) • Mileage reimbursement (IRS standard) Heartline Staffing is a Strategic Talent Partner with Purple Cow Recruiting. Requirements
- Minimum 1 year Skilled Home Health experience REQUIRED (HARD STOP)
- OASIS experience REQUIRED (HARD STOP)
- First-time travelers NOT accepted
- BLS REQUIRED (in hand at submission)
- Must have reliable transportation
- Must be comfortable with independent field work
- OR PT license REQUIRED (pending accepted, must be active prior to start)
