About The Position

Services SFT is looking for a Social Worker for a mandate at the CISSS de la Gaspésie – CLSC de Sainte-Anne-des-Monts. The Social Worker intervenes with clients living at home to assess their needs, promote their home maintenance, and offer psychosocial support to users and their loved ones.

Requirements

• Member in good standing of their professional order.
• Bachelor's degree in social work.
• Minimum 2 years of experience as a social worker.
• OCCI training.
• Training to conduct psychosocial assessments in the context of a protection plan.
• Valid driver's license.
• Vehicle required.

Responsibilities

• Assess the needs of users at home.
• Develop and implement adapted intervention plans.
• Offer psychosocial support to users and their caregivers.
• Perform psychosocial assessments in the context of a protection plan.
• Collaborate with the interdisciplinary team and network partners.
• Ensure clinical follow-up and file writing.
