About The Position

Provides comprehensive care coordination, supporting a holistic and coordinated approach throughout the continuum of care. This includes assessing, planning, implementing, and evaluating nursing care for Trauma patient population. The Trauma Nurse Navigator supports patients within the hospital, through discharge and outpatient. Responsibilities include reduction of risk factors and strengthening resiliency by supporting trauma patients and their families that are cared for at Froedtert Hospital. Work in conjunction with staff to provide resources for physical and mental health healing. Navigate the electronic health care record, including documentation of patient and family support. Provide in person visits and phone calls to patients. Other duties as assigned.

Requirements

  • Minimum of 3 years of clinical nursing practice experience in a related setting/specialty required.
  • Bachelor's degree in nursing is required.
  • Nursing assessment skills.
  • Creative solution-based problem solving.
  • Critical thinking skills and judgment.
  • Analytical skills to identify unusual circumstances that could put the organization or the patient/family at risk.
  • Strong customer service and interpersonal skills necessary to interact with patients, families, care team members, case management, etc.
  • Highly organized with the ability work independently and to prioritize responsibilities.
  • Knowledge of local resources and system navigation.
  • Knowledge and skills in Microsoft software applications.
  • Effective written and verbal communication skills.
  • Knowledge of managed care.
  • Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).
  • American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent.

Nice To Haves

  • A minimum of 5 years of clinical nursing practice experience in a related setting/specialty is preferred.
  • Master's degree in Nursing or related field, preferred.
  • Computer skills including EHR navigation, data management, presentations, excel, and report writing preferred.
