Responsible for providing safe transportation and supportive assistance in accordance with the individual’s functional ability to patients and guests in a timely, competent manner and communicating important information relevant to overall care plan to the multidisciplinary team. Responsible for van and escorting passengers during transport to and from pick up to delivery site. Follows established schedules and make adjustments to meet unanticipated changes; determines the safest and most efficient pick-up and delivery routes while considering needs of participants and requirements of clinic and therapies; maintains narrative of route. Safely carry out one-person transfers of passengers to and from vehicles and inside vehicles. This includes ensuring all passengers wear safety belts, wheelchairs are locked into position and correct use of ramps for loading and unloading patients. Maintains and ensures Oxygen supply and use is provided to patients as directed by clinical staff during transport. Follows regulatory processes to identify and report grievances and service requests with prompt reporting and appropriate escalation. Responsible for transporting medications, DME, and supplies for patient use with adherence to chain of custody for controlled substances and regulatory reporting. Responsible for administering first aid and CPR during transport, following emergency guidelines. Promptly reports patient or guest-related concerns noted in home or van. Documents according to program policy including accidents and incidents. Inspects vehicles and maintain them in a clean and safe condition. Maintains daily documentation of passenger logs, mileage, service records, etc. Offers support services to additional departments as needed, including transporting documents to post office, bank, etc. Provides support in the department.
