About The Position

Immanuel Pathways PACE® is seeking a Transportation Lead to work at Pathways Southwest Iowa located at 1702 North 16th Street, Council Bluffs, IA 51501. The schedule for this position is Monday through Friday from 7:30 am to 4:30 pm, with flexible start and end times depending on transportation needs. Pay will be based upon experience, certifications, skills and education. The overall purpose of this job is to assist the Transportation Supervisor/Manager with key functions such as overseeing scheduling appointments and routes, transporting participants, and providing coverage for the transportation supervisor when gone. In addition to transporting participants, the responsibilities of this job also include transporting meals, durable medical equipment, pharmacy, and any other items deemed necessary. Is responsible for overseeing the cleanliness and safety of vehicles assigned to drivers daily. Responsible for communicating any safety issues to the Transportation Supervisor/Manager. Be capable of, and experienced in, transporting individuals with special mobility needs. Supports and lives out Immanuel’s Mission and CHRIST Promises.

Requirements

  • High school diploma or equivalent is required.
  • One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid Driver’s License in the respective state in which he/she resides and in Iowa, must have a chauffer’s license or ability to obtain one within 14 days of employment.
  • Must have an acceptable record from the respective state’s Motor Vehicle Department in which the individual’s license was issued.
  • Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).
  • Hiring is contingent on successfully passing a DOT Medical Exam and obtaining DOT Medical Examiner’s Certificate.
  • Applicants must be currently authorized to work in the United States on a full-time basis.

Nice To Haves

  • One (1) year of commercial driving experience is preferred.

Responsibilities

  • Follows assigned schedule and makes necessary adjustments to meet unanticipated changes, while considering participant needs and Pathways PACE Center requirements. Communicates these changes promptly to dispatch.
  • Maintains and completes daily required records i.e., Pre- and Post-Trip Inspections, gasoline purchases.
  • Immediately reports, to Transportation Coordinator, any vehicle or cell phone operating problems, malfunction, or incidents out of the ordinary routine.
  • Operates equipment safely and in compliance with vehicle licensing regulations.
  • Oversees to ensure vehicles are neat and in clean condition.
  • Ensures vehicle equipment is maintained and operating within manufactures recommendations, replacing supplies as needed.
  • Oversees the dispatching of drivers during scheduled routes and appointments.
  • Functions as a member of the Interdisciplinary Team (IDT). Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Oversees and ensures the daily work schedules for each Driver and/or Aide is communicated.
  • Communicates Interdisciplinary Team decisions, participant changes, plan of care changes, and updated information to transportation staff daily.
  • Ensures the scheduling, coordination and communication of the driver route assignments of Immanuel Pathways Program.
  • Oversees transportation dispatching and routing services, dispatches paratransit vehicles as appropriate.
  • Overseas the creation of daily manifests; maintains, modifies and updates schedules to improve services.
  • Performs a wide variety of clerical duties including answering phone and data entry.
  • Maintains accurate records, logs and files on transportation and paratransit services.
  • Processes transportation requests in a timely manner.
  • Supports the scheduling of participants for transportation to and from the Immanuel Pathways Center, medical doctor or specialist appointments, and activity outings.
  • Works with Transportation Supervisor/Manager to resolve scheduling problems for the purpose of ensuring all routes are covered in a safe, efficient, and timely manner.
  • Supports the coordination with clinic of contracted services appointment times and schedule.
  • Supports the coordination with day center for escort needs for appointments.
  • Assist with the development and monitoring of transportation routes to improve safety and efficiency.
  • Communicates with Transportation Supervisor/Manager and drivers daily to be updated on participant conditions/changes, plan of care changes, or Interdisciplinary Team requests.
  • Assists to ensure the schedule is documented in the electronic medical record.
  • Responsible for Immanuel Pathways participants-passengers during transport, providing care in a manner which ensures participant satisfaction.
  • Transports participants to and from medical/specialist appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care.
  • Provides physical assistance to each participant when needed as he/she boards and debarks from the program’s vehicles.
  • Transports meals, durable medical equipment, pharmacy, and any other items as deemed necessary for participant.
  • Functions as a member of the Interdisciplinary Team when appropriate.
  • Communicates with Transportation Supervisor or Transportation Manager daily to be updated on participant conditions/ changes, plan of care changes, or Interdisciplinary Team requests.
  • Follows safety policies and guidelines through: Complying with OSHA (Occupational Safety and Health Administration) guidelines, Implementing Exposure Control Plan, Complying with Emergency Preparedness Plan, Using proper lifting and push/pull techniques, use gate belt assistance and participant transfers.
  • Perform other duties as required or requested.
