About The Position

The Transplant RN Care Coordinator reports to the Transplant Programs Manager within the Transplant Center. This role plays a critical part in supporting transplant patient care and program operations by applying advanced clinical judgment, consultation, education, and evidence-based practice to promote high-quality care for transplant patient populations. The position requires a highly motivated, adaptable RN who is comfortable working independently, managing competing priorities, and responding effectively in time-sensitive situations. Responsibilities include supporting the training and development of new team members and maintaining current knowledge of applicable UNOS/OPTN and CMS regulatory requirements. Training for this position requires a minimum of 3 months and follows a hybrid model, combining on-site training at the downtown Charleston campus with remote training alongside an assigned preceptor. Following completion of initial training, the position transitions to a remote work model; however, team members must remain flexible and available to attend monthly in-person meetings in Charleston and to participate in additional on-site training, meetings, or departmental activities as needed.
