About The Position
This role is for a Transplant Coordinator (Full-Time, Day Shift, 40 hours/week) within the Liver Transplant department. The primary responsibilities include evaluating potential liver transplant recipients, working with the transplant team and referring physician to activate and follow patients on the waiting list, maintaining MELD scores for waitlisted patients, admitting potential transplant recipients and donors into the hospital for transplant, participating in cross-training for post-transplant recipient management, and assisting in the operation of pre-transplant evaluation clinic hours.
Requirements
- Bachelor's Degree
- Registered Nurse
- Minimum of three years experience with hepatic and/or transplant patients
- Strong communications and writing skills
- Proficiency with computer applications
Responsibilities
- Evaluate potential liver transplant recipients
- Activate and follow patients on the transplant waiting list
- Maintain MELD score for waitlisted patients
- Participate in transplant clinic
- Participate in the on-call rotation
- Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital.
