As Transplant Coordinator, you will coordinate the Transplant Program. In this role, you will support patients undergoing complex abdominal surgery and intestinal transplant procedures before, during, and after transplantation, evaluate potential transplant candidates, arrange care during transplant, and communicate patient conditions and updates. You will serve as an educator, providing patients and families with information related to the entire transplant process. You will also be responsible for scheduling appointments, communicating patient assessments over the phone, and collaborating closely with surgeons and the broader care team. This position is an excellent opportunity to not only enhance the treatment experience but also gain new skills, knowledge and expertise in the field. A caregiver in this position works a hybrid schedule, with two days remote and two days in office, from 7:00 a.m. -- 5:30 p.m. (four ten-hour days per week, with one rotating day off each week).

Requirements

  • Current state licensure as a Registered Nurse (RN) or Registered Dietitian (RD)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
  • One year of experience in acute care, medical/surgical, or ambulatory setting within a hospital environment

Nice To Haves

  • Bachelor’s of Science in Nursing (BSN) within three years of hire date
  • Two years of RN experience

Responsibilities

  • Assess potential transplant candidate patient with the transplant physician.
  • Facilitate plan of care and symptom management per physician recommendations.
  • Provide education to patients and family members regarding all phases of the transplant process.
  • Coordinate scheduling for follow-up appointments.
  • Review and collate medical information received from referring physicians for patients seeking or undergoing transplant evaluation.
  • Facilitate tissue typing of recipient and donor and arrange timing and collection of pre-testing before transplant.
  • Serve as a contact person for peri-transplant evaluation for physician and patient/family and/or insurance companies.
  • Prepare abstract and present case to selection committee.
  • Place patients on the national waiting list for transplantation.
  • Coordinate all activities related to organ acquisition, including evaluation of the donor workup information.
  • Coordinate with hospital unit personnel in the discharge of post-transplant patients and living donor patients.
  • Provide immediate assessment and delivery of care in a timely and cost-effective manner.
  • Monitor post-transplant patient status (e.g., physical, emotional, complications, S&S rejection, infection, etc.) and communicate to providers appropriately.
  • Assess recipient compliance with medications and treatment plan and collaborates with transplant team to initiate changes in drug/treatment plan as indicated
