About The Position

Responsible for the case management of the post-transplant renal patients. This position is designated as an on-site, patient-facing role. In-office presence is an essential function due to the need for real-time collaboration with providers, direct interaction with patients and caregivers, and secure handling of health data in compliance with HIPAA.

Requirements

  • Graduate from an accredited nursing program with AND, ASD, or BSN.
  • Active RN License in NC and in good standing.
  • CPR and First Aid Certified
  • Valid driver’s license and reliable transportation
  • Understand the ethics of confidentiality and the ability to maintain confidentiality of sensitive information.
  • Oral and written communication skills.
  • Interpersonal skills.
  • Manual dexterity
  • Maintain a positive and professional attitude in all aspects of work from patient care to interaction with co-workers and physicians.
  • Demonstrated computer skills including working knowledge of Microsoft Word, Outlook and Email, and applicable EMR/Practice Management systems.

Responsibilities

  • Develop care plans for post-transplant patients.
  • Pre-transplant tracking.
  • Participates in Chronic Care Management for post-transplants patients.
  • Maintain accurate medical records by documenting patient communication, changes in intervention per medical provider, referral calls, pharmacy calls, and triage calls in the EMR.
  • Assist with patient calls, hospital calls, prescription refills, and lab/test results.
  • Patient assistance and prior authorizations for post-transplant patients.
  • Back up infusion department.
  • Responsible for providing care to the following patient populations: adult and geriatric post-transplant patients.
  • Gathers and maintains information on post-transplant patients to determine the needs to be met prior to the doctor’s exam, to include: Obtain patient vital signs, Review allergies, prescription medications, and over the counter medications, Labs and radiology.
  • Other duties as assigned.
  • Notify appropriate provider on any issues that may arise during post-transplant clinic.
  • Provide nursing interventions as appropriate for patient.
  • Carries out doctor’s orders via verbal, written, electronic health record, and messaging platform.
  • Enters orders for providers and other staff.
  • In-basket management office communication, refills, forms, orders, and labs.
  • Provides health education to patients and care givers.
  • Patient assistance form completion.
  • Assists with prior authorization of medication and procedures.
  • Triage patients and phone calls from providers, patients, and care givers.
  • Chronic care management of transplant patients.
  • Schedules procedure, infusions, and labs as ordered.
  • Monitors yearly health screenings for transplant patients.
  • Lab result monitoring and tracking.
  • Monitor hospital census for transplant patient and assist with post discharge appointments.
  • Responsible sending out new graduate packets to transplant patients.
  • Assists with social services transportation voucher and forms.
  • Supports the Acute transplant unit in transitioning patients back to regular clinic.
  • Provides coverage for clinic staff.
  • Assists with Infusion department.
  • Oversight and assist pre-transplant coordinator with transplant referrals.
