The Transplant Coordinator I, RN - Kidney Acquisition is responsible for developing and maintaining nursing practice standards of care for a specific transplant population. This role serves as a liaison, providing consultation and education to support these standards. The coordinator identifies and resolves clinical issues related to transplant patients, collaborates with staff, patients, and families on evaluation and discharge planning, and maintains documentation for transplant candidacy and follow-up care. They coordinate communications with the outpatient transplant population, including clinic visits, community referrals, consultations, treatments, and hospital admissions. Additionally, the role involves developing and coordinating a system for patient flow from referral through follow-up and referral back to the community, and participating in research and process improvement activities. The position requires independent coordination of outpatient care, including participation in clinic visits, analysis of diagnostic tests, monitoring patient compliance, reviewing data, and intervening in patient care problems. The coordinator also manages donor procurement and recipient preparation, maintains the UNOS waiting list, communicates with transplant team members, and coordinates the admission and preoperative preparation process. They participate in policy and procedure development, create educational programs for patients, families, and staff, and serve as a resource person. The role involves communicating patient and family problems to the multidisciplinary team and assisting with interventions. Keeping abreast of regulatory policies (Hospital, CMS, OPTN/UNOS) and clinical education in transplant and evidenced-based research is crucial. The coordinator independently facilitates the evaluation and listing process within regulatory requirements. They maintain communication with referring physicians and insurance case managers, providing necessary information and education. Database management, including the UNOS waiting list, TIEDI forms, and internal databases, is a key responsibility. The role oversees and supervises support staff, functions independently within clinical skills and established processes, and collaborates with the multidisciplinary team across the continuum of care. Participation in quality meetings and QAPI activities is expected, along with adherence to all hospital policies and maintaining a professional attitude.
