About The Position

The Transplant Coordinator I, RN - Kidney Acquisition is responsible for developing and maintaining nursing practice standards of care for a specific transplant population. This role serves as a liaison, providing consultation and education to support these standards. The coordinator identifies and resolves clinical issues related to transplant patients, collaborates with staff, patients, and families on evaluation and discharge planning, and maintains documentation for transplant candidacy and follow-up care. They coordinate communications with the outpatient transplant population, including clinic visits, community referrals, consultations, treatments, and hospital admissions. Additionally, the role involves developing and coordinating a system for patient flow from referral through follow-up and referral back to the community, and participating in research and process improvement activities. The position requires independent coordination of outpatient care, including participation in clinic visits, analysis of diagnostic tests, monitoring patient compliance, reviewing data, and intervening in patient care problems. The coordinator also manages donor procurement and recipient preparation, maintains the UNOS waiting list, communicates with transplant team members, and coordinates the admission and preoperative preparation process. They participate in policy and procedure development, create educational programs for patients, families, and staff, and serve as a resource person. The role involves communicating patient and family problems to the multidisciplinary team and assisting with interventions. Keeping abreast of regulatory policies (Hospital, CMS, OPTN/UNOS) and clinical education in transplant and evidenced-based research is crucial. The coordinator independently facilitates the evaluation and listing process within regulatory requirements. They maintain communication with referring physicians and insurance case managers, providing necessary information and education. Database management, including the UNOS waiting list, TIEDI forms, and internal databases, is a key responsibility. The role oversees and supervises support staff, functions independently within clinical skills and established processes, and collaborates with the multidisciplinary team across the continuum of care. Participation in quality meetings and QAPI activities is expected, along with adherence to all hospital policies and maintaining a professional attitude.

Requirements

  • Nursing BSN
  • 1 year Transplant Coordinator experience (Heart Transplant, Lung Transplant, Liver Transplant, Kidney/Pancreas Transplant) required
  • For Heart Transplant - One year of cardiology experience
  • For Lung Transplant - One year of pulmonary experience
  • For Liver Transplant - One year of abdominal transplant experience
  • For Kidney/Pancreas Transplant - One year of abdominal transplant experience
  • OR 2 years Organ Procurement experience and 2 years critical care experience required
  • Excellent communication skills, including the ability to speak, read and write English proficiently
  • Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including whole blood collection, and apheresis procedures, in accordance with administrative and clinical policies, regulations and procedures
  • Registered Nurse - RN (CA Board of Registered Nursing)
  • Active CA RN license
  • Basic Life Support (BLS) Healthcare Provider from American Heart Association
  • Fire Life Safety Training (LA City) - If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only)

Nice To Haves

  • Nursing Master’s Degree in Nursing or in progress, strongly preferred
  • Master’s Degree in Nursing may be in lieu-of Bachelor’s Degree in Nursing

Responsibilities

  • Independent coordination of outpatient care of the transplant patient
  • Coordinate donor procurement and recipient preparation
  • Participate in the development of policies and procedures pertinent to the transplant program
  • Participate and develop appropriate educational programs for patients, families, nursing staff, and medical staff

  • Independently facilitate the evaluation and listing process of the potential transplant patient within the regulatory requirements of CMS and OPTN/UNOS
  • Maintain communication with referring physicians and insurance case managers, supplying necessary information and records
  • Provide education regarding the transplant evaluation, listing and organ allocation process
  • Database management including UNOS waiting list, TIEDI forms, internal departmental database, and data collection during the transplant process
  • Oversee the assignments delegated to support staff, evaluate their performance, and provide direct supervision
  • Function independently within clinical skills and established processes for patient flow
  • Collaborate with the multidisciplinary transplant team to provide care for the transplant patients across the continuum
  • Attend department quality meetings and participate in department QAPI activities
  • Adhere to all hospital policies including attendance, punctuality and dress code
  • Maintain a professional and positive attitude at all times in both patient and office settings
  • Perform other duties as assigned
