About The Position

The Transplant Coordinator I, RN - Kidney Acquisition is responsible for developing and maintaining nursing practice standards of care for a specific transplant population. This role serves as a liaison, providing consultation and education to support these standards. The coordinator identifies and resolves clinical issues related to transplant patients, collaborates with staff, patients, and families on evaluation and discharge planning, and maintains essential documentation for transplant candidacy and follow-up care. Key responsibilities include coordinating communications with the outpatient transplant population, such as clinic visits and treatments, and developing a system to manage patient flow from referral through post-transplant follow-up. The position also involves participating in research and process improvement activities related to transplant patients.

Requirements

  • Nursing BSN.
  • 1 year Transplant Coordinator experience (Heart Transplant, Lung Transplant, Liver Transplant, Kidney/Pancreas Transplant) required.
  • 1 year Transplant Coordinator experience required for: Heart Transplant - One year of cardiology experience; Lung Transplant - One year of pulmonary experience; Liver Transplant - One year of abdominal transplant experience; Kidney/Pancreas Transplant - One year of abdominal transplant experience OR 2 years Organ Procurement experience and 2 years critical care experience required.
  • Excellent communication skills, including the ability to speak, read and write English proficiently.
  • Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including whole blood collection, and apheresis procedures, in accordance with administrative and clinical policies, regulations and procedures.
  • Registered Nurse - RN (CA Board of Registered Nursing) Active CA RN license.
  • Basic Life Support (BLS) Healthcare Provider from American Heart Association.
  • Fire Life Safety Training (LA City) - If no card upon hire, one must be obtained within 30 days of hire and maintained by renewal before expiration date. (Required within LA City only).

Nice To Haves

  • Nursing Master’s Degree in Nursing or in progress, strongly preferred.
  • Master’s Degree in Nursing may be in lieu-of Bachelor’s Degree in Nursing.

Responsibilities

  • Independent coordination of outpatient care of the transplant patient by participating in clinic visits, analyzing diagnostic tests, monitoring patient and family compliance, reviewing data and initiating appropriate action, and identifying and intervening in patient care problems.
  • Coordination of donor procurement and recipient preparation by maintaining the UNOS waiting list, communicating with transplant team members regarding donor and recipient status, and coordinating the admission process and preoperative preparation.
  • Participation in the development of policies and procedures pertinent to the transplant program.
  • Participation and development of appropriate educational programs for patients, families, nursing staff, and medical staff.
  • Serving as a resource person to all staff involved in the care of transplant patients and their families.
  • Communicating identified patient and family problems to multidisciplinary team members and assisting with appropriate interventions.
  • Keeping abreast of regulatory Hospital, CMS, OPTN/UNOS policies, and ongoing clinical education in transplant and evidenced-based research.
  • Independently facilitating the evaluation and listing process of potential transplant patients within the regulatory requirements of CMS and OPTN/UNOS.
  • Maintaining communication with referring physicians and insurance case managers, supplying necessary information and records, and providing education regarding the transplant process.
  • Database management, including maintaining the UNOS waiting list, completing required TIEDI forms, managing internal departmental databases, and collecting and maintaining data during the transplant process.
  • Overseeing assignments delegated to support staff, evaluating their performance, and providing direct supervision.
  • Functioning independently within clinical skills and established processes for patient flow.
  • Working in collaboration with the multidisciplinary transplant team to provide care for transplant patients across the continuum.
  • Attending department quality meetings and participating in department QAPI activities.
  • Adhering to all hospital policies including attendance, punctuality, and dress code.
  • Maintaining a professional and positive attitude at all times in both patient and office settings.
  • Performing other duties as assigned.
