As a Transition to Community Peer Support Specialist Team Lead, you will provide support and supervision to the Peer Support staff and provide therapy to their consumers. The purpose of the Peer Support Specialist Transition to Community Living service is to enhance the consumer’s ability to function and achieve their goals while residing in an independent living/community setting. The ratio of supervisor to CPSS is 1 supervisor to 8 Peers. The Peers will have a maximum caseload of 15 clients. The individuals being served are diagnosed with a Mental Health and/or Substance Use disorder. In addition, the individuals will be living in Permanent Supportive Housing and participating in the Transition to Community Living program. The intent of the service is to increase an individual’s ability to live successfully in the community by maintaining tenancy. Provide individual, family, and group therapy using evidence-based practices aligned with client needs and diagnoses. Supervise and support Certified Peer Support Specialists, including administrative oversight and quality assurance of services. Collaborate on recovery-oriented, person-centered planning and ensure timely assessments and documentation. Facilitate therapeutic and skill-building groups, and ensure access to community resources as needed. Maintain clinical documentation, including treatment plans, progress notes, and required reporting. This description outlines the general scope and level of work for this role.
