About The Position

NFI Massachusetts Inc., a leading non-profit human services organization, is seeking a Transitional Specialist to join our dedicated team at the Intensive Residential Treatment Program (IRTP) in Westborough, MA. Our IRTPs are short-term, secure facilities that serve male and female adolescents with serious mental illness or severe emotional and behavioral disturbances. Referred by the Department of Mental Health (DMH), these youth receive comprehensive treatment and care in a structured environment designed to support their journey to recovery. As a Transitional Specialist, you will play a key role in helping youth successfully navigate their transition from our program to life in their home or future communities. This includes supporting them for up to three months post-discharge to ensure continuity of care and connection to community services. You’ll also help youth develop essential life skills like independent living, making this a truly impactful and rewarding position.

Requirements

  • Bachelor’s degree in social behavioral sciences or related field, or equivalent experience.
  • High energy and strong interpersonal skills to work well independently and in a team.
  • A valid driver's license and a reliable vehicle are essential for this role.
  • Must be fully vaccinated or willing to get vaccinated against COVID-19, per state regulations.

Responsibilities

  • Identify and connect youth to community providers and services post-discharge to ensure ongoing support.
  • Coordinate visits to potential placements like group homes and schools, acting as a bridge between the client and external service providers.
  • Help youth immerse in their community, supporting them in real-life experiences and navigating services.
  • Assist youth with tasks like obtaining a driver’s license or applying for disability, preparing them for greater independence.
  • Stay connected with the youth for up to three months post-discharge, visiting them in the community and collaborating with providers and families.
  • Lead a weekly life skills group to help youth build confidence and practical abilities for the future.
  • Receive training as a Human Rights Officer and advocate for the rights and well-being of the youth.
