About The Position

Rite of Passage Team is hiring for a Transition Service Coordinator at South Florida Youth Academy, in Miami Beach, Florida. South Florida Youth Academy is comprised of multiple co-located residential commitment programs serving high-risk male youth aged 14 to 18 who require behavioral/mental health, substance abuse, and major disorder services. The programs utilize evidence-based practices to provide comprehensive care, treatment, and supervision. Youth will also receive psychotherapeutic services, including individual, family, and group counseling while at the program.

Requirements

• Have a bachelor's degree or better.
• Be able to pass a criminal background check, drug screening, physical, and TB Test.

Responsibilities

• Supervises a caseload of approximately 14-24 students.
• Assists with facilitating court hearings, Probation Officer, visits, student visits and/or home passes, and meeting with students to review progress.
• Primary program liaison between student, parents/legal guardian, and placing agency officials.
• Provides monthly progress notes and weekly student updates.
• Coordinates activities with other program staff.
• Participates in assessments and treatment plans.
