About The Position

The Transitional Liaison Registered Nurse helps identify patients who may qualify for hospice care and supports the hospice referral, admission, and documentation process. The role assesses patients’ physical, emotional, social, and spiritual needs; coordinates transitions and services with care teams; and meets with patients and caregivers to discuss goals and complete initial assessments. The nurse explains hospice services to patients and families and educates staff and referral partners about hospice eligibility and the transition process.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 3 years of experience in a clinical care setting.
