About The Position

The Transitional Liaison Registered Nurse identifies patients who may qualify for hospice care and follows the hospice referral, admission, and documentation process. The role assesses patients’ physical, emotional, social, and spiritual needs; coordinates patient transitions and services with care teams; and meets with patients and caregivers to discuss care goals and complete initial assessments. The nurse explains hospice services to help patients and families make informed decisions and educates staff and referral partners on hospice eligibility and the transition process.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• At least 3 years of experience in a clinical care setting.
